The filling in and sending of the form entails authorising the processing of the personal data entered, pursuant to the discipline and the guarantees established in Law no. 675/96. The concerned party enjoys the rights of article 13 of Law no. 675/96. Sending the data entails that the processing of the same is authorised by the concerned party. Processing of personal data is understood to mean its collection, recording, organisation, storage, elaboration, modification, selection, extraction, comparison, use, interconnection, blocking, communication, broadcasting, cancellation and distribution, or any combination of two or more of said operations. The questions asked and the relevant answers will be stored solely for the purposes of making up a file for internal use, with guaranteed respect of the professional duty of confidentiality.
